Health Insurance

Eligibility: All full-time regular (FTR) and part-time regular (PTR) employees are eligible for health insurance.

Medical Insurance
Carrier: CareFirst Blue Cross-Blue Shield
Type of Benefits: Blue Preferred (PPO) or HMO

Always ask if the provider accepts Blue Cross when you make an appointment. To receive maximum benefits, visit a Blue Cross in-network provider in the state where services are provided. To find an in-network provider, visit www.carefirst.com. Out of network coverage is also available. Specific details can be obtained from ProSol's Human Resources office. Prescription Drug Plan
Carrier: CareFirst Blue Cross-Blue Shield
Type of Benefits: Participating Pharmacy Plan

To receive maximum benefits use a Blue Cross participating pharmacy. Most PCS participating pharmacies are considered Blue Cross participating pharmacies; however, please ask before having a prescription filled. Present your Blue Cross identification card and your prescription to the pharmacist. You will then need to sign a form for the pharmacist. The co-payment for prescriptions is as follows:

Blue Cross designated non-preferred brand name prescription – $35
Blue Cross designated preferred brand name prescription – $20
Generic prescription – $10

Dental Insurance
Carrier: Metlife
Type of Benefits: PPO Dental

Dental insurance is offered free of charge to all employees and their families. o receive maximum benefits use a participating dentist. Present your identification card when you go to the dentist. There is a yearly deductible on all work except preventive maintenance.

Vision Insurance
Carrier: Spectera
Type of Benefits: Member Doctor Plan

To receive maximum benefits use a member doctor. Employees are eligible for an eye exam every 12 months. Pay $10 to doctor at time of exam. Employees receive a $110 frame allowance every 12 months. Discounts are available for lenses. The patient must pay for any extras that are requested. No identification card will be issued.

Eligibility: All FTR and PTR employees are eligible once the 90 day introductory period is complete.

Disability Insurance

Short Term

60% of weekly earnings, to a maximum of $750 weekly benefits, for 11 weeks maximum.

Long Term

FTR

60% of monthly earnings, to a maximum of $3,000. Buyup LTD benefit is 67% of monthly earnings, to a maximum of $6,000.

Retirement Plans
Basic Retirement Plan
Carrier: ADP 401K
Eligibility: FTR and PTR employees after 90 days of service.
Contributions: ProSol contributes 4%

Profit Share Plan

ProSol offers a profit sharing plan for full time and part time employees who have completed one full calendar year of service. The employee has to be employed as of January 1st and remain an employee through December 31st, working a minimum of 500 hours during that time period in order to be eligible for a plan contribution. The plan administrator provides regular statements to all eligible employees. The CEO at his discretion may determine the amount to be distributed each calendar year.

Tuition Assistance

ProSol encourages educational/professional development for all employees. Employees may choose to participate in the quarterly book reviews, college courses, professional training, etc.
